NO PURCHASE OR PAYMENT OF MONEY IS NECESSARY TO ENTER OR WIN THIS SWEEPSTAKES. A PURCHASE OR PAYMENT OF MONEY WILL NOT IMPROVE THE CHANCES OF WINNING.
The Diaper Giveaway for New and Expecting Parents (the “Sweepstakes”) is intended for residents of Kentucky and Indiana and shall only be construed and evaluated according to U.S. law and the laws of Kentucky and Indiana. Do not enter this Sweepstakes if you do not have primary residence in Kentucky or Indiana or are otherwise ineligible to enter in accordance with these Official Rules at the time of entry. These Official Rules apply to all individuals who submit a Sweepstakes entry and are eligible to enter and win (referred to in these Official Rules as “you” or “participant”). By entering the Sweepstakes you acknowledge and agree to these Official Rules. Do not enter the Sweepstakes if you do not agree to these Official Rules.
HOW TO ENTER: Register online at https://qualmedicaresearch.com/diaper-giveaway/. NO PURCHASE NECESSARY. PURCHASE DOES NOT INCREASE ODDS OF WINNING. Void where prohibited. By participating, you consent to Qualmedica’s use of your personal information for the administration of the Sweepstakes, or in any other manner consistent with these Official Rules. All entries become the property of Qualmedica and will not be returned.
DATES OF SWEEPSTAKES: To be valid, your entry must be received by 11:59 CST on the Sweepstakes End Date specified at https://qualmedicaresearch.com/diaper-giveaway/. The potential winner will be selected at random from all online entries within ten days after the Sweepstakes End Date. Qualmedica will contact the potential winner by email or phone using the information provided at the time of entry.
PRIZE AND PRIZE CONDITIONS: The Prize that may be awarded to the eligible winner is 200 count newborn diapers. If potential winner cannot be reached within three (3) calendar days from the first notification attempt, or if a potential winner is not in compliance with these Official Rules, then such person shall be disqualified, the Prize will be forfeited, and an alternate winner may be selected. Qualmedica reserves the right at its sole discretion to substitute the Prize with another prize of equal or greater value. No substitution or transfer of Prize or election of cash in lieu of Prize will be permitted. In no event will more than one Prize be awarded.
SPONSOR/ADMINISTRATOR: The Sweepstakes is sponsored and administered by Qualmedica Research, LLC, 920 Frederica Street, Suite 1010, Owensboro, KY 42301.
ELIGIBILITY: The Sweepstakes is open to residents of Kentucky or Indiana who are at least eighteen (18) years of age or older at the time of entry and who are expecting a newborn baby or have a newborn baby under two weeks of age at the time of entry. Only one (1) entry per person. Employees of Qualmedica Research, LLC and members of the immediate family of such employees are not eligible to enter and win. The term “immediate family” includes spouses, siblings, parents, children, grandparents, and grandchildren, whether as “in-laws,” or by current or past marriage(s). Qualmedica reserves the right to limit or restrict, upon reasonable notice, participation in the Sweepstakes to any person at any time for any reason.
ODDS OF WINNING: Odds of winning depend upon the total number of eligible entries received throughout the Sweepstakes period.
OTHER REQUIREMENTS: The winner will be required to drive to one of two of Qualmedica’s research sites (located at 5015 Plaza East Blvd., Evansville, IN 47715; and 920 Frederica Street, Ste. 1010, Owensboro, KY 42301) to pick up the Prize or contact Qualmedica at (812) 205-2475 to arrange for shipment of the Prize. The winner will also receive information regarding pediatric clinical trials. Transportation to one of the research sites is the responsibility of the winner if claiming the Prize in person. Qualmedica will pay for shipping to ship the Prize to winner’s residence in Kentucky or Indiana if shipment is arranged in lieu of in-person pickup. The winner must claim the Prize within fourteen (14) days of being notified. Failure to claim the Prize in person at one of Qualmedica’s research sites, or by contacting Qualmedica to arrange shipment within such fourteen (14) day period will result in forfeiture of the Prize and another winner will be selected.
USE OF PERSONAL INFORMATION: To enter the Sweepstakes you must provide your first name, last name, email address, phone number, zip code, and your baby’s due date or birth date. By entering the Sweepstakes, you agree that Qualmedica may use the personal information you submit with your entry to contact you in the event you win the Sweepstakes. You also agree that Qualmedica may use the personal information you submit with your entry to contact you with information about future sweepstakes and pediatric clinical trial studies, unless you have indicated you do not want to receive these communications at the time of entry or you have subsequently opted out of receiving such communications. Qualmedica will use the information you provide about your baby’s age to verify your eligibility to enter and win, and to identify pediatric clinical trial studies that may be of interest to you. Qualmedica may send communications about future sweepstakes and pediatric clinical trial studies to you by e-mail or hard copy, including U.S. mail. Participants may opt out of receiving communications from Qualmedica about future sweepstakes and pediatric clinical trial studies when registering for the Sweepstakes or subsequently by following opt-out instructions provided with the communications. Opting out of receiving communications from Qualmedica will not impact a participant’s chances of winning the Sweepstakes. Qualmedica will not disclose personal information obtained from participants in the Sweepstakes to third parties except with the participant’s consent or as permitted or required by law.
RELEASE: By participating in the Sweepstakes, each participant and winner waives any and all claims of liability against Qualmedica, and its employees and agents, for any personal injury or loss which may arise out of participation in the Sweepstakes. Participants acknowledge that Qualmedica has neither made nor is in any manner responsible or liable for any warranty, representation, or guarantee, express or implied, in fact or in law, relative to the Prize or the Sweepstakes.
MISCELLANEOUS: Qualmedica may amend the Official Rules or discontinue the Sweepstakes at any time. For a copy of the Official Rules or information regarding the Sweepstakes winner, mail a written request with a stamped, self-addressed return envelope to Qualmedica’s headquarters located at 920 Frederica Street, Suite 1010, Owensboro, KY 42301. ANY ATTEMPT BY A PARTICIPANT OR ANY OTHER PERSON TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, QUALMEDICA RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PARTICIPATE TO THE FULLEST EXTEND PERMITTED BY LAW.
COMPLIANCE WITH LAW: The conduct of the Sweepstakes is governed by the applicable laws of the United States of America, as well as the Commonwealth of Kentucky, which take precedence over any rule to the contrary herein. Qualmedica shall follow the applicable laws for conducting the Sweepstakes.
